Humble Beginnings
Doreen M. Lewis founded Community Health Associates in 2004 in Yuma, Arizona. Doreen’s vision was to provide mental health care services to all individuals in need, including the underserved and the uninsured.
Our dedicated staff and teams strive to deliver the vision and level of care that Doreen Lewis always intended. At Community Health Associates, we are dedicated to serving the mental and physical health care services needs of individuals throughout all communities.

Outpatient Psychiatric Health Services
Our providers will assist you in achieving your mental health goals. Looking at your history, family history, current situation, and needs together we work with you to develop a treatment plan to get to your optimum functioning. We offer video conferencing (telehealth) for those who have transportation difficulty as well as assistance for things like pharmacy coordination, medication management, and primary care coordination, to name few. Physical Health Services
While focusing on prevention our medical providers have the expertise and training needed to diagnose and treat a wide spectrum of medical issues for all ages. While focusing on prevention, our staff focus on management of behavioral health symptoms. Specializing in addiction, we also work with every behavioral health issue. We address the member, the family, and the member’s support system.
