Mission
We believe it is every consumer’s right to receive care in the least restrictive setting, to live independently when possible, to be employed and to be educated. Our staff are committed to helping consumers fully exercise their rights, and learn how to use local resources, and systems of care, to meet individual and family needs. We also believe each our clients and consumers must have decision-making authority and “choice” regarding their care.
Humble Beginnings
Doreen M. Lewis founded Community Health Associates in 2004 in
Yuma, Arizona. Doreen’s vision was to provide mental health care to all
individuals in need, including the under served and the uninsured.
Our dedicated staff and teams strive to deliver the vision and level of care
that Doreen Lewis always intended. We are dedicated to serving the mental
and physical health care needs of individuals throughout all communities.

At Community Health Associates it is our goal to be a leader in developing and providing innovative, culturally competent, customer-directed, quality behavioral health services and to form community partnerships that make a difference in the lives of the people we serve. This results in us living out our vision which is to give our communities a choice and our consumers a voice.
