Justice Involved Services
Community Health Associates is working collaboratively with Adult Probation Departments, in partnership with AHCCCS “Targeted Investment Program” to operate the nation’s first Justice-Involved Integrated Outpatient Clinics designed to:
- Reduce the fragmentation between acute and behavioral health care
- Increase efficiency in service delivery for those with behavioral health needs
- Improve health outcomes for individuals with physical and behavioral health needs
Our TIP sites are dedicated to integrating primary care and behavioral health services, for the purposes of better coordination of preventive and chronic illness care, for adults transitioning from the Criminal Justice System.
